The correct answer is C. Certain topics are considered sensitive and should be discussed with minors only after parental permission is obtained. The physician should be prepared to answer the boy™s question, but only after the child™s parents have agreed. Note that the physician™s talk is not part of the usual curriculum and does not carry the sanction of the governing bodies of the school.

Passing off the issue to the teacher avoids the issue and does not answer the question (choice A). Although no direct answer should be given at this point for fear of upsetting community sensibilities, the process that will allow the discussion to happen can be initiated.

Opening up the question to the rest of the class is a nice presentation technique, but hints at a discussion that should not happen without parental permission (choice B). By this response, the physician makes an implicit promise that he cannot fulfill.

Although parental permission is needed, the physician should not shy away from this discussion (choice D). This is not merely an issue for parents, but one that can and should be discussed openly with children as they age.

Choice E starts the discussion by allowing each child to tell the others what they have heard or learned. Again, the physician should be willing to have this conversation, but after checking with community sensibilities.
