i agree with ya all but he is immunocompromised patient,taking out his cath will put him on more risk then being cath-in status and infection and so treat with broad spectrum drugs until culture result comes and then u gv this patient definate treatment.
so i will go with 3 only.

i have few questions. the catheter which is producing acute inflammation, what is the point of keeping a contaminated device there? and another point is dont we need to culture to be sure what is it? if he is to be catheterized, it can be done with a new sterile catheter after starting antibiotic therapy. isnt it?

how can 3 be the answer man. the catheter is probably the source of the catheter. u might as well take the catheter out and cut its tip and send that for culture
