How do program directors know about a profile change in ERAS say visa or ECFMG certification...?
Are we notify all the programs we applied to again about the change in the profile ?

when u do change in ur profile eras post office upload the new profile and then all the programs u applied to will be sent the new profile to be uploaded just keep ur profile up to date

That's not true, actually you can't make any changes once you submit your CAF. The only things that are udpdated frequently are your ECFMG status report (in case you get certified later on) and your usmle transcript (in case a new score comes in). Answering to your question, there's nothing you have to do since your certification is already available to the programs in case they are interested in you. GL
