MSPE letter help - erg

Hello partners,
Just learned that I need to have MSPE letter for the ERAS application... Slightly disappointed..
I am very old graduate, graduated in Soviet Union (majority of you probably don't even remember this country) 20+ yrs ago.
Is this paper absolutely necessary? If so, is it's format must be the same as recommended on ERAS web site?
Can anyone send me a sample of what you have/did? Any russians out here?
