The most common cause of thrombocytopenia in hospitalized patient is Heparin.
HIT Heparin Induce Thrombocytopenia: The immune system produce antibodies vs a complex, this complex is made of heparin + platelet facto IV + IgG Ab. The result is platelet activation leading to clot formation (paradoxical thrombosis) and low blood platelet levels leading to bleed. Rx Argatroban, Lepirudin, Danaparoid and Hirudin. They are direct thrombin inhibitors that bind to the thrombine active site. They don’t require antithrombin III for activation.
