Cholelithiasis...pigmented gallstone is seen most in infection of the biliary tract such as this case and also it can be seen in patients with chronic RBC hemolysis,alcoholic cirrhosis,

release of beta glucoronidase by injured hepatocytes nd bacteria

macronodular change due to severe liver injury (post infectious) leading to hepatic necrosis.
