What date to fill in as:
1.)Date you completed requirements for the final medical diploma
2.)Graduation Date

Is it before internship or after? I passed exams in Dec 2003 and completed my internship in Dec 2004.

I have recently submitted the form for USMLE STEP1&2. You need to enter on or after "Dec 2004".

no, write the date u completed requirements for ur diploma thats is completion of internship and for graduation write the actual graduation date. for example for me it is jan 2003 and dec 2003 respectively.

sorry should have been more clear, graduation date is the day or month you recieve ur diploma may differ with university. for me took a year
