These questions are really words games. Everyone is pretty clear what is the right thing to do, but you may not get the right answer. for this question, of course D. But if the question is the most effective treatment, then it is probably A (without immediate). Correcting dehydrationvolume depletion and electrolytes is always the first thing to do.

The electrolyte imbalance has to be corrected before the surgical option [A] It is like correcting or lowering a high BP before surgery, any other correct option has to wait until this is achieved.

I think that in any question ABCs come first.Airway, Breathing, Circulation-blood , fluids if they ask what is the most appropriate next step in management and the patient is unstable and resuscitative measures have not been done in the stem question.
