An elderly with gross hematuria develops aphasia and right hemiplegia, followed shortly by coma and death after 2 days in the hospital. What is the most likely cause of the hematuria?

i thinkits 3cuz hematuria is the most frequent presenting abnormality.correct me if i am wrong.

2. aphasia and right hemiplegia are due to infarction. This might due to embolism, kidney is suffered too.

Option 3 seems to be good but your reason for 2 is convincing. It is hard to choose which one if we do not see the real picture. This Sunday, I will work on a website to allow us to upload image for discusssion. Hopefully, it will function on Monday.

I would go with renal cell carcinoma, the CNS problems come after the hematuria and it is possible to have a renal cell met to the brain causing his cns symptoms...it is a tricky one
