Tb meningitis has very decreased glucose levels like bacterial, but the wbcs are linfo

TBM has moderately low glucose but not very low as seen in pyogenic.

it is difficult to differentiate cryptococcal from TBM . Only thing that can be done is staining. India ink for cryptococcci ( it might be negative in few patients), so if negative and suspision high, do cryptococcal antigen test.
Tbm can have cobweb formation on gross examination, PCR for mycobacterium tuberculosis can be done in addition to acid fast staining. Cultures can be done but reports take 4-6 weeks and patient's require immediate treatment. |
