go to oasis and log on and the click ROD services.
They are pretty quick in dispatching the documents.

i will cost you $15. and in approximately 15 days they will send you the doc that you submitted to ERAS, except those that you waived your rights to see.

if you live outside usa then you will also have to pay fedex charges
