Guys can anyone of you advice while waiving a LOR should the writer send just the LOR cover sheet or also the document submission form which we send them. As far it is mentioned in ERAS we should include both LOR cover sheet and document submission form. What have u all done guys? Is it both or just the LOR cover sheet? Thanxz

As far as i know , If u waive ur rights to c lor, than attach only cover sheet. If u do not waive than attach document submission form. I have also done same thing , i attached only cover sheet to lors which i waived.

Thanxz scabies. Actually my LOR writer says do not include the document submission form. But ERAS says any documents sent to them should include document submission form. So i have planned to mail both the LOR cover sheet and document submission form.
