Let's clarify a few things here:
There are 2 types of fellowships- Clinical and Research fellowships
You can work in the united states following both...........but to do that, you must have completed a residency in the united states (If you want to work after clinical fellowship) BUT in the case of working after research fellowships, completing a residency in US is not mandatory.
Note however, that you cannot work in a clinical capacity after a research fellowship (you can only work in a research capacity) even if you had earlier completed a residency in ur own country.
You should see both fields (clinical and research) as not interchangable. While they potentiate and feed off each other, u can't replace one with another.
