Hi..i have been asked by a program to bring the completed Univ Appln to the interview.Have a few doubts in that!plz help!

sorry..
1.month/yr of matricualtion at medical school...do i write my grad date or the date i finished final yr..i mean exclude the internship?
2.what does he mean by Electives completedor planned?
3.Do we need to paste the PS again on this from ERAS??
4.Interview scheduling..do i fill up the date i am going to for the interview??
Any suggestions r welcome!Thanks n good luck to all!
