H1 visa is transferrable, there's no doubt. The only problem is on switching to a different program on second year residency.

adonis...it is extrememly difficult to get good LoRs from the residency u will leave to join a new one...
happens only in genuine cases...like say...your family has moved somewhere and u want to move too...
and then too ...LoRs from your peds prog will not be really useful when u apply to IM.....and if u give your old LoRs...then u will definitely be questioned about it in the interview...an
how will u convince the PD of an IM prog that next year you will not fancy surgery and leave their prog for it.....
just coz u have H1 for peds doesn't mean u will get H1 for IM next year
don't be so cheap...IM progs are numerous in number and easier to get into that peds prog...don't go into peds if u don't want to......IM programs sponsor H1s more readily than peds programs
