what do we need to give for proof of internship to the pd. I have letter from the head of department, but what else do we need

Your medical school transcript should have enough details on what you did including your grades during internship.
Don't be obcessed by providing proofs to PDs. There is a requirement list posted on websites of each program. THat should be your priority
