the only thing which favors papillary is that the fibrous tissue with blood vessels in it maybe cores of papillae but the description is not accurate here.

As for follicular carcinoma and a non functioning follicular adenoma, you can't tell the difference by FNAC. Both will appear as cold nodules and both will show neoplastic follicular cells.

For medullary carcinoma, not usually diagnosed by FNAC. You need to see amyloid by congo red or by H&E section of a biopsy.

I will go with A because in Robbins Basic Path is the only one referred as "most frequently cold nodule"

D. Follicular cell variant of Papillary Thyroid Carcinoma, here follicular cells is a distracter., this papillary carcinoma variant demostrates at histologic follicular cells formed empty follicules, rossets,tubules and papillary structures also called solid balls of follicular neoplastic cells
