Breast-fed osteoporosis???? - sarim

1-If breast feeding frequently and for prolonged period of time it could cause osteoporosis.
what's the mechanism????

2-How does hyperprolactinemia(e.g. in prolactinoma) can lead to osteoporosis(mechanism) and since we have high prolactin serum levels during pregnancy then why it does not affect the bones?

Please give it a try.....

answering both q by saying that in both situation body should be in negative Goasotropin status seconderyt to hyper prolacinemia and this aggrevate osteoporesis. this is just my thought.

correction goasotropin to gonadotropins from troph cells especially FSH.

1. breast milk is generally known to be rich in all vitamins and mineral necessary to infant till 6 months of life with the expect ions of vit d and k. so it is important to start weaning(introduction of solid food) at 6 months so infant can receive vit d from food. so hence prolonged breast feeding would cause rickets...

2. because high prolactin levels inhibit pulsitile realese of LH and FSH, so granula cells cant produce estrogen and low estrogen causes osteoporosis(like in menopause) because there is a decrease in osteoblastic stimulation...

oh no!! sorry..i thought you were asking why does prolonged breast feeding cause osteoporosis in children... but yeah 2 answers both q anyway. this is also the basis of lactational amenorrhea.

thanx for the explanation

1-it's the "Lactational Suckling Reflex" that inhibit GnRH--->decrease in LH n FSH---> decrease Estrogen release from Granulosa cells.
DECREASE ESTROGEN:--> overexpression of "RANK-receptors" on osteoclasts. and RANK-ligand secreted by osteoblasts will bind to these receptors promoting osteoclast activity--->bone resorption.

(Parathyroid hormone have receptors on osteoblasts which communicate with osteoclasts thru RANK- ligand to cause bone resorption)

....decrease ovulation(there for can not get pregnant during this period of lactation)
....lactational amenorrhea
... dry vagina(can cause painful intercourse)
... decrease libido

as drkdeepti explained increase in prolactin will also contribute in above effects by decreasing estrogen levels.

2- during pregnancy estrogen levels are high(cause of placenta) and that's what stimulate the release of prolactin.

and also eventhough Prolactin is high during pregnancy it does not produce milk synthesis cause high Estrogen and Progesterone levels block it's function of lactogenesis which kick start at child birth when estrogen n progesteron go down.
