thanks meti... i knew it from CK UW... acc to it adverse prognosis if :
1)ph < 7.3
2)INR > 6.5
3) creatinine > 3.4
4) blood lactate > 3.5
5) po4 >1.2

- also, if clear hostory of large vol acetaminophen ingestion as in this case, directly give NAC.
-if no clear idea abt amount ingested, check plasma levels at 4 hrs ( nomogram) and if toxic levels are found ,then give NAC.
