common application form doubt?..plz help - bj

1)onpage8 :if i am ecfmg cert do i need to mention it here ?
2) on page9 for state licences do i have to mention the state licence i have in my country(karnataka,India) or,does it mean only US state license
3)regarding the documents plz correct me,is it photocopy of diploma,medical transcripts and deans letter and original lors or is it originals for all the above
4)should i have to wait tfor sept1 for requesting transcripts or can i do it now?
plz plz ur help is much appreciated

hi,

why don't you just contact NRMP/ERAS. What you applying for with Common Application Forms, as most the residency is now through EARS or it is ERAS form you talking about?

Anyway,

1) I do not know
2) state license for USA only. I assume you don't have one. Just write None or N/A.
3) Always photocopy of documents, not the original one.
4) I do not know
