I think what MTB says is when you have classical sign of acute epidydimis still just rule out Testicular tortion by US. not when its obvious that its TT. then directly go to surgery

Yeah I agree,although USS Doppler is helpful to see the blood flow, but clinical feature is typical for torsion, emergency surgery is TOC to save the testis.
