u can apply for fellowship directly instead of residency but u won't be board certified. if u plan to practice obgyn then except obgyn residency no other way. u can do any fm or internal med ,surgery first year then transfer to second year position next year, if u get, that is possible. but not sure anyone will want to take img with intern year in other speciality in second year.

Krishi,
there is another way to get license in the specialty. (I kno for Ortho for sure, my frnd is doing that)
3 fellowships in a row and then get a license in the specialty.
Otherwise as posted earlier, u can get obgyn. one of my IMG frnd tried- didnt get, but AMG got it.
So chances are 50 % to get OBGYN. But be careful, this field is very risky in US. Instead IM FM is more good and decent life.

as people said you could do Ob-Gyn residency or FM residency. Following FM residency you can do a fellowship in "women's health" which is basically ob-gyn stuff....
Following Ob-Gyn residency you can do a reproductive endocrinology fellowship, which deals with infertility.
I don't think there is any way to deal with ob-gyn related issues/infertility in IM, so that should be your last choice if you are really interested in Ob-Gyn.

@ krishi
you dont have option for the inferitlity fellowhip from IM.
From FM it is women's health --- but you might not get job for the infertility on basis of that.
your best bet is the Ob/gyn residency.
Yes you can do the direct fellowship as some one mentioned. But in that case there are few problems
-- you can get license to practice that speciallity , BUT IT IS LIMITED TO THAT INSITITUTION ONLY
and they dont pay good as oppose to the regular one
--- You have to get your abroad OB/GYN degree approved by the lic. board where you are and they have specific requirements like the written transcript of what you did back home.
-- OB/GYN is having second highest malpractise insurance after ortho, so which ever place you work might not proivde malpractise insurance for your abroad qualification and with malpractise insurance one cant practise here
