How do you know they have tb?

Do an abdominal exam for abdominal pain, vomiting, diarrhea, jaundice, uti and pelvic pain.

routine physical exam should include [always] chest lung and abdomen. Any additional exam depends on the diagnosis and differential diagnosis you have in mind. But abdominal exam is not necessary in psychiatric cases, because you have the series of neurological exams to do.
