your query about osteomylitis ,which test to first.

i just read in usmle world inf disease q.

if its acute like 2/3 days ...go for bone scan forst/techenium scan.cos it is very sensitive.show result when its early.

if its >2 week..then go for X.ray.cos x.ray show changes after 2 week.

about culture yes take cultures and start emperic antibodies without waiting for cultures results./

emperic Rx =oxa/naficillin plus Aminoglycoside/3rd gen cephalosporin
