mine would be
1. Depression
2. Medical Causes (not that psychiatric diseases are not medical conditions - lol)
hints for depression "he has a low energy level. He feels he is just "too old." "
so start with an open ended question - check depression - ofcourse u'll do other tests also - this is the USA - u won't let the guy go home without pricking his cubital vein and handing him a bill worth few hundred dollars which his insurance will decline to pay him.
