1-yes you can but I don't advice you to do that from same author to different programs in the same specialty. You may mention the name of program when you fill the dsf or give it a number like medicine-1. You may send from same author to different specialties as many people do that.
2-you will see them in MyEras as you send them in dsf
3-it depends for what you are using this letter for, if you have the letter already in your hands you don't need the cover letter just fill the dsf, if you are waiving your rights to see the letter and the writer is sending it directly from his office then you may mention on it which specialty like medicine or surgery, but if you want the LOR to be used for multiple specialties, keep it blank or put "residency programs"
