i will say b..coz her age is unlikely for cardiac problems..n she has a neck mass to guide u towards probable diagnosis..if TSH is normal then search for other causes

If Pt is Symptomatic (i.e. Palpitations.....EKG..) -> Give Propanolol 1st to immediately control Heart Sx's

If not Symtpomatic (i.e Palpitaions) Investigate Neck mass --> TSH Initially

First you do TSH with Free T4, then Ultrasound -will tell you solid or cystic and then FNA with Ultrasound guided biopsy...and then SCan..so enjoy..
