kaplan checklist for CIS - mccallan

ok when you write patien note follow this order 1) D/D and Diagnostic test 2) physical 3)HX

when you write DD write from most probable to least, and for diagnostics test most simple to complex.. first write down forbidden physical exam followed by relevent test

Write the CC in patients own word and for HPI always use medical terms. for example if the patient say burning urination,shortness of breath, removal of appendix just write on patient not as dysuria, dyspnea, appendectomy ,polypectomy... always use medical terms...in HPI

dont use complete sentences and waste time.. just write 28 yo F patient c/o pain in middle of chest for last 2 hours, constant,sharp 8/10, gradually progressive,nonradiatin,no alleviating aggravating factoers. Has h/o nausea vomiting,, blah blah.. SO DONT WRITE FULL SENTENCES

USE abbreviations as much as possible

Write all positives and if you have time towards the end write down the negatives keeping thr differentials in mind.
for example for a case fatigue.. you need to write no h/o cold intolerence,constipation,skin changes, change in weight appettie, bowel or bladder..
(here you need to think of DD hypothyroidism and cancer)

for physical write down all postives and negatives. in the above case you need to mention
HEENT: no pallor, NECK: no thyromegaly, just mention about chest,heart,and abdomen, and ext: DTR

Finally to get good score in patient note you must have taken a good history and noted evrything briefly on your clipboard.. then you can easily transfer them to PN (so practice this part very well its very important)

go through all case in FA and UW and find out what POSITVES AND NEGATIVES that you have to wirte in the hisotry and physical exam

hope it helps i am not good at patient note so thats all i can advis you for the time being

in the physical also mention about hair, skin edema for case of fatigue..

do we have to get 19 to 20 out of 23 to pass CIS.......? really? where do u get the info? pls let us know?

the check list i directly copied from kaplan 5day course material
and the score details i got it from a friend who attended the kaplan 5day course

he said that the passing score for CIS has been increased to 90%. there for if you calcuate 90% of 23 it would be around 19-20

is it hard to follow all 23??

ok there are 96 flash cards i am just gonna write down the case presentation.. please find out the DD and Diagnostics testss okay i dont have the patience to write evrything

1)27 yo M with Non traumatic DVT
2)28yo Irish M with unexplained abdominal pain for "years" and negative workup
3)42yo caucasian G3P# with c/o frequent urination and accidents

5)27 yo F married for >3yrs having difficulty getting pregnant for last 18 Months
6)63 yo m c/o favorite meal is not appealing anymore has loss of taste (dysgeusia)
7)28 yo c/o that his girl friend says his breath "stinks"
8)23 yo M IVDA with back pain and arthralgia
9)32 yo M autimechanic with 2days of low back pain
10)38 yo M executive with 10days of fever and negative out patient workup
11)37 yo male with h/o alcoholism comes in with 3 episodes of vomiting blood
12)18 yo M olympic gymnastic with 1 yr of diarrhea

13)24 yo medical resident returns from relieg mission in philipines with diarrhea and weaknes
14)24 yo M with 1 year diarrhea weight loss and skin rash
15)52yo F with c/o food "gets stuck in my chest"
16)44 yo M comes in for hypercalcemia noted on routine labs during his annuel checkup
17)33 yo F wiith headaches and palpitation, BP 190/110
18)30 yo F with join pain , weakness and fatigue for 1 year
19)45yo obese M c/o polyuria and confusion
20)51 yo M alcoholic with acute confusion
21)28 yo F with co swollen glands in her neck for 1 month
22)58yo F with dry mouth for 1 year
23)21 yo college football player with c/o "i feel like i am druck when i walk"( concussion)
24)22yo F with recurrent mouth sores

will type the rest tomorrow.. (you can buy this book usmle step 2cs: complex cases if you want.. for more cases) good night
