the q doesn't say the guy has UTI. like u said, He;s 72, probably has urinary tract obstruction

The goal of the question: to make sure u understand that bacteremia arising from E.coli almost always originates from UTI. so many things can predispose someone to bacteremia including BPH, fecal incontinenence...etc.
This old dude perhaps had BPH thus---> obstruction--> bacteremia. gl
