First question . Answer is D - both Beta blocker and ACEI
Beta blocker because it reduces mortality in CHF by 60%
ACEI because it reduces mortality in any CHF when EF is less than 45%
In the first question patient has an extra indication for ACEI that is Diabetes mellitus also. If a patient has DM and HTN, the first choices are either HCTZ or ACEI. If the patient has DM nephropathy and HTN, the first choice is always ACEI.

Second question Ans. B
This question is different than previous question. Beta blocker gets priority over lisinopril if both are listed in the choices seperately. Beta blocker in post MI patients with CHF, reduces mortality by 60% where as ACEI in CHF with EF < 45% reduces mortality by 20%
So, beta blocker gets priority over ACEI here.

second Q we need to add lisinopril and cavidolol there must be another choice missed.

according to acc/AHA:
stage A:no structural heart disease with risk factors to CAD, no symptoms like Q1, treat with life style modification and ACEI/ARB. they lower mortality.
stage B:with structural heart disease like previous MI, valvular lesions, no symptoms, life syle modification, ACEI/ARB and beta blocker Carvidolol superior to metoprolol and bisoprolol. like Q2.
stage CConfusedtructual heart disease with CHf symptoms,ACE, Betabocker, Diuretics, Digoxin.
stage D: refractory CHF symptoms to maximal medical management, like A+B+C and mechanical assist device like ICDs,heart transplantation,continuous iv inotropic infusion, hospice care in selected pts.
