a thank you, It will be my second attempt, during my first attempt I did good score for CIS but fail in ICE, I think it was due to my Patient notes and physical exams.

GOOD. Good style.

BUT most important, you have to feel COMFORTABLE with wathever style you are using. Don;t just used because someone used and passed. time yourself, practice and if you feel comfortable, go ahead.

HERE IS HOW "CAN" YOU CAN WRITE PN!!! Its called "B.U.L.L.E.T... S.T.Y.L.E" format. Helped me a lot while cuz I typed.

"23 y/o M c/o fever, Chest pain, chills, nonproductive dry cough x 2 days. insidious onset, progressively worsened, constant. Right sided Chest pain, 2/10, pleuritic type, dull, nonradiating. No alleviating, aggrevating, precipitating factors. CP worsened with deep inspiration. NEGATIVES (-): no N/V, no diarrhea/constipation, no abd pain, no wt loss/night sweats/hemoptysis. no urinary/GI/sleep problems

PMH: + h/o type I IDDM, no surgeries, no previous hospitalizations
All: NKDA
MEDS: Insulin shots
SH: no Tobacco/ETOH/Illicit Drugs. no IVDA hx
SxH: Sexuallly active with girlfriend. no unprotecte sex with mult sex partners.
FHx: Mother alive with NIDDM, high cholesterol. HTN. Father not known.

VS: tachypniec, tachycardia, febrile, (102 F, 125/80mmHG, 25/min, 110/min)
GA: Pt is lethergic, responsive,
HEENT: nostrils clear b/l, no erythma, edema, exudates. Oropharynx clear. no oral thrush. no erythema, edema or purulent exudate. No LAD.
LUNGS: +BS throughout, +crackles at lung bases bilat. No wheezing no rales, no rhonchi. -kusshmauls breathings (!!! Pt hx says IDDM. just mentioned it. They'll be impressed!!!) Clear to percussion throughout.
HEART: regular rate & rhythm, normal s1s2, no murmurs, no rubs, no gallops
Extr: no pitting edema. No rashes noted.


DDx

pneumonia: community acquired vs atypical
URI exacerbation.
Influenza infection

Work Up

CXR; CBC w/ Diff ct
Blood Glucose Levels; HbA1c
Urine Ketone levels
Electrolytes: Na/K/Cl/HC03/PO4
Monospot test,as needed; Strep Test,as needed"

I made this case up as I typed to help you guys. But you get the idea!. Its quiet different from FA & UW format I think.

I flunked my last exam because of the PN ,I need to thank you because people tend to be surprised when I tell em that failed only because of that

I have a couple of questions
If you run out of time to do something for eg CVS in a case of stroke
Do you still mention it
or do you not mention it

Secondly with respect to the aforesaid mentioned case I have only a few days left and I cant seem to find the kaplan cs book in India do u have a digital copy
