I've heard that's pretty hard to find fellowship programs that actually sponsor H1.... Anyway you might always convert H1 into J1 for fellowship I guess, but I am actually not sure if that is feasible...

ok since we are at this, i wanted to know something since a while. what if i enter a program on a JI and eventually end up with a GC within the 3 yrs period, would i still have to go back to my country or work in underserved areas? sorry i am misleading your original post...

Yes. Unfortunately yes. Unless you marry a US citizen who files a "claim" saying that your having to move is giving him discomfort (I don't know the exact definition, but this exists), in that case you won't have to do the waiver.
