In AIDS patient the ring enhancing lesions are of two types

A:Toxoplasma
BTonguerimary CNS lymphoma
Differentiation is by anti toxoplasmic therapy.
--If lesion starts decreasing after two weeks = Toxo
--If the lesion size ,no change = Lymphoma

angleman, u r right. I think the bactrim prophylaxis and single lesion makes toxo very less likely.

But who knows he is taking Bactrim(which is for Toxo also) of PCP as prophylactic medicine not some other one like dapsone or Atovaquone ?

Misshyd ,dapson and atovaquone have unreliable efficacy in prophylaxis of Toxo. Secondly ,these are never used alone (if at all). Harrison does not recommend them in prophylaxis of toxo, alone or in combination (Ref : Harrison's manual of medicine).

"Medication Summary
Currently recommended drugs in the treatment of toxoplasmosis act primarily against the tachyzoite form of T gondii; thus, they do not eradicate the encysted form (bradyzoite). Pyrimethamine is the most effective agent and is included in most drug regimens. Leucovorin (ie, folinic acid) should be administered concomitantly to prevent bone marrow suppression. Unless circumstances preclude using more than one drug, a second drug (eg, sulfadiazine, clindamycin) should be added. The efficacy of azithromycin, clarithromycin, atovaquone, dapsone, and cotrimoxazole is unclear; therefore, they should be used only as alternatives in combination with pyrimethamine. The most effective available therapeutic combination is pyrimethamine plus sulfadiazine or trisulfapyrimidines (eg, combination of sulfamerazine, sulfamethazine, and sulfapyrazine). These agents are active against tachyzoites and are synergistic when used in combination.

the reference you posted is for treatment not for prophylaxis. Treatment of Toxo should never be with anything other than pyrimethamine.

Primary prophylaxis for Toxo can be with cotrimoxazole ( bactrim), dapsone or Atovaquone which are also the drugs used for PCP prophylaxis
