The lipid profile does not look impressive even after 6 months dietary modification, so will have to start on meds, preferably fibrates but alcohol cessation should be done too.......?

male more than 45 ---1 risk factor
Smoke---2nd risk factor. He is drinking for past 10 years---1 pint----Wow HDL should be MORE THAN 55 in real world. But TG >500---definetely gets priority. Have seen 100 or alchoholics----no body had TG like this. So from clinical experience---i wud go for Fenofibrate -----but I could be wrong. And believe me he is not gonna stop drinking

I think it is D because they have listed all the drugs that may help lower Triglycerides and bring up HDL so its hard to pick one .

to me this is not a very intelligent question. Patient at presentation had high triglycerides. He is given dietary modification. This modification includes reducing alcohol. how come he comes back after 6 months saying that he's drinking a pint of vodka a day thinking this is good for his good cholestrol???? yes it might increases his HDL but it worsens his hypertriglyceridemia. In any case i think this patient needs first to reduce his alcohol intake.
