from uw, i learnt that if modified weils criteria is >4 and pt has no contraindication for antocogulation you directly give heparin before doing CTA,but if pt has any contraindication then u do CTA first then IVC filter.
i am assuming we are always supposed to confirm diagnosis before putting IV filter!

Ya i think for q3 you would diagnose it first with spiral ct of chest before doing anything else. Also the last answer i think is subphrenic absess as it has referred pain to the shoulder.

Why q4 is A? why not B? what does it mean brief incontinance?

It means adult diapers. so it would be the least invasive method amoungst the options given so least chance of infection.

I guess q8 is E, Because although she is a splenic but she doesn't have any respiratory symptom , also she has tenderness on epigastr. I think it is Abscess after splenic surgery and her shoulder pain is referral from diaphragm .
