Jovanat to diagnose Lupus you have to fulfil 4 out of 11 criteria for Lupus. We can't be sure on that. Besides lupus won't cause hepatosplenomegaly, will it?. Plus Lupus causes symmetric arthritis. She has pain in her wrist and knee. ....My doubts; may be wrong.

this q is vague and the only thing which is more in favor of EMC is the palpable purpura; everything else goes with lupus, including GNitis AND less commonly Nephrotic syn. This Q is good but has incomplete information for STEP1 USMLE purposes. The mnemonic to remember the 11 symptoms is 'SOAP BRAIN MD' and yes HSM is uncommon in SLE.
