Here is my two cents for you, and I hope that you will spend it wisely.
Why should you be judged or evaluated by the "commoners" that are non medical professionals. Doctor. when you see patients, who are your patients, are they doctors? When you get paid as a physician, who pays you, fellow physicians? I don't particularly think so. Do these common people as you eloquently stated have the right to understand the care being provided to them by i) a non native speaker (which in any culture may be a problem)
ii) by an arrogant health care provider who has no regards for the pt's pt's wellbeing
This exam was initiated as a way to improve COMMUNICATION between patients and physicians which was getting to a point of outright shame. As far as this exam is concern, fellow physicians measure or critique your clinical skills, and the "commoners" that will be paying your salary will measure your communication skills. It is that simple; the client is always right, so to speak.
American exams measure different levels of a broad and vast knowledge, and I believe this is one of the only systems that is fair all over the world in this aspect. The issues of SPs laughing behind ones back is a very ridiculous statement.
This is the sort of attitude that cause the public to cry "foul" in the first place, and here we are being tested for communication. The ability to listen very carefully measures a lot of things, and put one in a different category of comprehension.
Very few of the people on this forum will let you treat them, with this kind of concept. We are all physicians too.
