ikeratoconus, i just wrote above that it is progesterone deficiency that is addressed by oc pill in pcos and the email that i got said it was erroneous in pcos area as they were attempting to cover all anovulation cycles together in 5 minutes. if its still bothering you, you should write to them if you need more clarity.

let me write gain if this will help:

simply the concept is


ANOVULATION CYCLES in Exercise induced amenorrhea or anorexia nervosa : associated with amenorrhea/ estrogen deficiency. Can have cystic ovaries due to chronic anovulation ( Cystic ovaries here is not PCOS) Osteoporosis occurs due to estrogen deficiency . Treatment is OC pills.

1. Anovulation cycles in PCOS : associated with irregular bleeding or amenorrhea. Associated with androgen excess, estrogen excess, progesterone deficiency. Risks are endometrial hyperplasia and cancer due to progestrone deficiency. Treatment is OC pills again

2. OC pill are therapy in both. OC pill serves different role in these two types ( both above conditions could be having cystic ovaries, what differentiates PCOS from other anovulatory cystic ovaries is exclusion of other causes of cystic ovaries first and androgen excess evidence)

OC pill provide estrogen in the first case. They provide progesterone in the second case.

Your exam q will be on OC pill as answer.

The patho-physiological basis for PCOS is androgen excess . The androgens may be converted peripherally to estrogen. This creates an estrogen excess state. Because of the cystic ovaries, there is also a failure of normal ovulation that consequently, results in progesterone deficiency. Therefore, estrogen remains unopposed and the estrogen in turn, is responsible for endometrial hyperplasia and possibly, endometrial cancer.
