enuresis qs, ask her since when child has been having this problem, if he had achieved bladder control before, ask her if there was some kind of new event in childs life like birth of a sibling, started attending school, shifted residence and how the child has adapted to this ie whether he is distressed or not
also ask about bowel movements to rule out constipation, ask about the fluid intake of the child especially sweet drinks, ask about burning micturition ie whether he appears to be in pain while passing urine, and whether he has been going to the toliet more frequently during day time ask her if he goes to toilet before bed time ask her how she has been dealing with this problem ie whether she gets angry, hit him ...... the usual stuff which we ask in all peds cases also ask her if there is somebody else in the house who helps in taking care of the child, whats his/her relationship with the child ie whether the child gets along well with him or not and also rule out any kind of abuse as bowel/bladder in continence can be due to physical/sexual abuse( as mentioned in Kaplan) other than this ask the usual peds qs like schooling, development, diet , immu and the whole lot........ as for counselling , i got this from sure4 notes Enuresis (Bed wetting) Counseling : "Mrs. Cole, kids with bedwetting usually undergo natural healing with age, but this problem needs tenderness. Punishing your child could worsen it - a much better technique would be to rather reinforce positive behaviour by praising and rewarding him if he stays dry while sleeping. I would advise you to monitor your child's drinking habits before going to bed and encourage the child to go to bathroom before going to bed. We also need to do some tests to rule out other conditions." as for dds-- diabetes, urinary tract infection, primary enuresis ( bladder control never achieved since birth) , spinal cord trauma ( ask this q in the history taking too for both bowel and bladder complaints) , psychiatric problems anything i forgot or mentioned incorrectly , Please do correct

first question must differentiate primary vs secondary ----- has she ever achieved control over bladder?
