once you feel the mass 3cm, Biopsy(fnac) the nxt best step, no doubt in 65 yr old

If female is older than 50 years of age it is cancer until proven otherwise. step1: Mammo step2: biopsy(fna), if cancer, Ductal carcinoma: core or excisional biopsy
Invasive ductal carcinoma: core excisional biopsy
Invasive labar carcinoma: core or excisional biopsy
Pagets disease: excisional biopsy + immediate radiation
Inflammation carcinomaSadmost deadly one)-->treat asap

Stage 1-tumor 2 cm or less---->tx:remove lump only + radiation
Stage 2&3-tumor + nodal/local envolvement------->modified radical masectomy + chemo or breast lumpectomy + chemo + radiation
Stage 4-mets(palative tx only)

Chemo drug option---> CMF(cyclophos,Mtx,Fluoro) or AC(adriamycin,cyclophos)
+Tamoxifen/Anastrozole if ca is estrogen receptor sensitive
+Trastuzumab if ca expresses the HER-2/neu oncogen(good px)

A 45-year-old premenopausal woman (G2P2) is evaluated for a lump she found in her left breast yesterday while taking a shower. Her first menarche was at age 13 years and first full-term pregnancy was at age 21 years, and she breast-fed both of her children. She has no family history of breast cancer, and no other medical problems.

Physical examination is normal except for the detection of a nondiscrete breast mass in the left breast. Results of mammography are normal.

Which of the following is the most appropriate next step in management?

A Follow-up appointment in 1 year
B Serum CA-15-3 or CA-27.29 measurement
C Breast MRI
D Positron emission tomography“CT
E Breast biopsy

you have to get mammogram, fine needle aspiration done before you say it is not a cancer. mammograph can still miss a good percentage of breast cancer, also, you do not want to get sued if she is found that she has cancer later.

that is the point here.

Mamo just for screen, if the lesion is highly suspected for malignancy, like this 66 female -3 cm breast mass, do biopsy first, do not wait for mamo.
