you are talking about lewy body dementia, very hard to treat.

Levodopa was most probably prescribed after the initial visit to decrease rigidity and bradykinesia, and improve overall functioning of the patient. The visual hallucinations, confusion and agitation on levodopa treatment are highly suggestive of dementia with Lewy bodies.

PD is a gait problem, levodopa should give to if the presentation is classic.
but the lewy presents differently from the beginning, we may try to give atypical antipsychotic medication..
