Current American Cancer Society screening recommendations for otherwise healthy women include annual clinical breast examination and mammography beginning at age 40 years and pelvic examination and Papanicolau test for all sexually active women (can be done every 2-3 years after 3 successive annual examinations with normal results in low-risk women). In this woman, who has a firstdegree relative who had colon cancer at age 50 years, current recommendations are for her to have her first colon cancer screening done at an age 10 years younger than the age at diagnosis of the affected relative. Normally, colon cancer screening is not begun until age 50 years, but the family history in this case should alter the recommendations. There is no standard role for chest radiography for screening, especially in a nonsmoker. Cancer antigen 125 is a serum marker for ovarian cancer and as yet has no role in screening, nor does routine ovarian or pelvic ultrasonography. Ultrasonography cannot currently be recommended for screening for breast cancer. Recent findings show that women who have a very high risk for breast cancer (greater 20% lifetime) should undergo screening MRI. These recommendations mostly include women who have an inherited gene mutation such as BRCA1 or BRCA2 or women who have had prior radiation to the breast region (most often for lymphoma).
