which one to download and how do i know who is the authorized official?
"Certification of Identification Form (Form 186) - Certification by Medical School Official"
or a
"Certification of Identification Form (Form 186) - Certification by Notary Public and Medical School Official."

i used the certification by notary public & med.school official but i cant remember the reason.i think u use that if you have graduated or if you were not applying from country of graduation or something.
just follow the instructions but the 2nd form sounds more like for graduates

thanks sammi but how do i know who is the authorized official? there is warning that if the signature of the med school authorized official does not match with ecfmg records then then the application will be invalid. worried about it.

Hi, In my opinion, if you want the form to be attested by school dean then do the first one and they will match the signature of your school dean or whoever is responsible in their file from other people who have written the exam . Its usually the dean or principal who does it.
Second one is if you are away from your medical school and you can go to a notary public and do it , both are valid .

yeah but have already listed those authorized to sign it eg college dean/provost etc.your medical school must have been signing this form before now so i am sure they know the rules and must have delegated the duty to someone.
in my school its strictly the college dean that signs it so my advice is
1)get it notarized first(if you are the US just walk into a bank with your id and ask for their notary official,they will sign it free even when u are not their customer)
2)send it to your college secretary through a friend or family member and he will direct you from there or ask your classmates that have done this b4

like presto said,if you are still in your country just go to your school secretary.if they identify you in person you will not need to notarize
