MEN II or III, medullary are calcitonin secreters and he has enlarged thryoid and h/o parent with thyroid cancer

MEN II a -Sipples -- MPP - Medulary Thyroid C, Parathyroid Hyperplasia, Pheochromocytoma

I would go with Calcitonin.....
