pt has fever,sore throat,no cough,lymphadenopathy,exudate,rapid strep negative,what next
1 do thraot culture
2 give penicillin
3 give azithromycin
4 discharge
5 reassure it is viral

2. Strep score here is 4 . why did they do rapid strep?

if strep test is negative- then do culture

if strep is positive, this is enough for Dx- treat with penicillin

no, next step in managing strep throat depends on strep score

Original Criteria (interpretation below based on these) - 1 point for each
Tonsillar exudate
Tender, anterior cervical adenopathy
Cough absent
Fever present

1. Strep Score 4 - Treat with antibiotics
2. Strep Score 2 to 3- Perform rapid antigen test
If Antigen test positive: Treat with antibiotics
If Antigen test negative: Throat Culture (Requires 24 hour minimum for adequate growth )most specific (99%). Sensitivity 90%. Not recommended as primary test due to 24 hour delay . Remember that “ve Rapid strep does not rule out Strep throat.

Strep Score 0 to 1
Provide Pharyngitis Symptomatic Treatment  salt water gargles, sucking candies, ibuprofen

In the question you posted, Strep score was 4. So, directly treatment
