why not cold antibody ? - newboy

60 y/o pt. with lymphoma complains of weakness, exertional SOB, easy fatigability, HGb-10, Hct-30, Ret. count -3.5, LDH-400,, + spherocytes, + Coombs test for IgG, most likely DX
1. cold Ab Hemolytic Anemia
2. hereditary sperocytosis
3. autoimmune hemolytic anemia
*4. warm Ab Hemolytic anemia

In cold ab hemolytic anemia, the hemolysis occurs only when the patient is exposed to a cold environment (or so I think i've read) Since the q doesnt mention anything like that, warm Ab hemolytic an wud be more likely.

cold antibody anaemia is caused by IgM antibodies and warm by IgG and our patient has IgG antibodies.

Autoimmune haemolytic anaemia may be due to:

warm reactive IgG autoantibodies
cold reactive IgM autoantibodies / cold haemagglutin disease
drug provoked haemolytic anaemia
complement activating IgG of paroxysmal cold haemoglobinuria
