A 34 yr old hiv pos man is brought to the er with hypotension,tachycardia , fever and s/s of severe shock. His blood culture is pos for aspergillus. Which of the following factors is crucial in the pathogenesis of this type of shock.

Well done cd45 n voice..The reason is macrophages respond to LPS of gram neg bact by producing TNF which sets into motion the release of other cytokines IL-1,6,8...generating the pic of septic shock.
I was confused with IF gamma but then depletion of T cells in AIDS explains the absence of this mech
Any inputs..have I faltered somewhere.

i'll go with..B,aspergillus ---extracellular pathogen,..T cell related..need IF gamma for cytotoxic CD8 action.in HIV CD4 count depletes not CD8....PLEASE CORRECT ME IF I'M WRONG!

But raptor isn't TH1 cell required for IF gamma prod.
