thru history u find out what is the cause, whether it is psychological or medical. If it psychological u tell the patient that it seems to be psychological but u might need to run few simple blood tests just to look for any medical cause and then u will have seesions to get over this psychological barrier.

If cause is medical then u tell the patient that I think it might be secondary to diabetes, ur previous surgery or trauma, we will need imaging tests or blood tests , when we have the results, we will discuss the cause, extent of disease and possible therapies. So we will meet after test results in one week and if u have any concerns, u let me know.
