35 y/o c/0 fatigue and weakness - jovana

1. A 35 “year old come to your office with complaints of increasing weakness and fatigue. He has noticed abnormal bruising and epistaxis. On admission he developed fever and pneumonia. The spleen is not palpable. Lab results reveal immature-appearing cells of the neutrophilic series. Auer bodies are seen on peripheral smear. The most likely diagnosis is:
a. acute lymphocytic leukemia (ALL)
b. acute myelogenous eukemia(AML)
c. chronic myeloblastic leukemia (CML)
d. chronic lymphocytic leukemia (CLL)
e. non-Hodgkin's lymphoma (NHL)

AUER rods are present in most cases of AML especially the promyelocyte class. There are 8 classes of AML, each with unique features. Acute promyelocytic leukemia is associated with a t (15;17) translocation and disseminated intravascular coagulopathy is common.

The correct answer is B-AML because Auer rod in a myeloblast is seen ONLY in acute myelogenous leukemia (not chronic, not lymphocytic, not monocytic, not in NHL)
Answer a “ALL is usually seen in a age brackets “kids and the patient is 37 y old.
Answer c-CML is seen in age brackets 45 y old or more and the pathognomonic is Philadelphia chromosome, NOT Auer rod.
Answer d-CLL is the most common leukemia in people over age 60 and present with massive splenomegaly.
Answer e. NHL usually present™s with night sweats and pruritis
