A 52-year-old white male who has untreated bronchogenic carcinoma comes to your office with a 1-week history of dyspnea, facial edema, and marked neck vein distention which has been progressive over the last 2 days. A chest roentgenogram confirms a mass in the right hilum with a small pleural effusion in the right hemithorax.

Immediate management should consist of

a.pericardicentesis
b.mediastinoscopy
c.exploratory thoracotomy
d.radiation therapy

D. first, if recurrent do balloon angioplasty with intravascular stent. Surgery may be palliative in those with benign cause of superior vena caval syndrome.

i think it should be mediastinoscopy...as u have to get a tissue diagnosis first...to find out if its radio or chemo sensitive before u start therapy...so should be bbbbbbbbbbb
